The logo was created, I believe, by Stanley Mouse and debuted at the Winterland shows, when the movie was filmed.

Augustus Owsley Stanely the Third, the Dead's soundman in the 60's designed the logo. He also brewed a good portion of the Frisco LSD back then.

I personally can't stand the Greatful Dead... but I can't argue the pure originality of the logo. (And notice fans from the previous "skull" logo, at least they DID SOMETHING to make this skull stand alone.)
